The beauty of a mommy makeover


The mommy makeover is one of the most common procedures plastic surgeons – including myself -perform. The mommy makeover is a combination of breast surgery and tummy tuck to restore the shape of the breasts and stomach to their pre-pregnancy states.

The breast surgery can either be a lift, a lift with implants placed or can involve surgery to the nipple areolar complex if needed. Some women only need the breasts repositioned after pregnancy. The milk engorgement, nursing, hormones and changes that occur for the breasts during pregnancy and nursing can make the breasts feel and look flatter and lower than prior to pregnancy. This is also something that naturally happens as we age, so there can be a combination of factors at play. In this case, a breast lift alone may be the only breast surgery needed.

Sometimes, projection of the breast is the major issue and an implant can be placed under the muscle or on top of the muscle to regain some of the projection of the more youthful breast. There are times when both a lift and augmentation are needed to achieve desired result.

One area that we do not talk about as much is the nipple areolar complex. The nipple itself can become elongated from nursing and hormone changes, and the areola itself can look larger once the breast has been stretched with pregnancy or weight gain. The areola itself can be reduced in size, as can the nipple. This can be done in combination with either a breast lift or a breast lift with augmentation, and can be performed alone also.

For the tummy tuck portion of the mommy makeover, there are also a couple of different options. The first is a mini tummy tuck that can be performed through a smaller incision like a C-section scar. This can be performed for women who have had C-sections or for women who have not. This is less invasive than a full tummy tuck, but not everyone is a candidate. A full tummy tuck is a longer incision than the mini tummy tuck incision and can incorporate an old C-section scar and revise it. Both of these surgery options can be performed in women with and without C-sections and both involve removing the excess skin on the abdomen. One area of the tummy tuck that most people are not aware of that I commonly do is a diastasis repair. The diastasis is the longitudinal fibrous tissue of the abdomen that separates the “six pack” of the abdominal muscles, and has commonly been spread by the stretching of the abdomen with pregnancy. When women say they cannot get their waist back after pregnancy, this is usually why. The fibrous tissue between the abdominal muscles has been spread out, and no amount of sit-ups will return the fibrous tissue to its original state. With a tummy tuck, I can go in and repair the diastases, essentially cinching the waste back down to give women back their natural hourglass curve.

Two Tummy Tuck Transformations by Dr. Timothy Janiga

With my line of work, often seeing is believing. That’s why today we are profiling two of our patients who underwent drastic tummy tuck transformations. These women have very different body types, so you can see how plastic surgery can benefit just about anyone.

Our first transformation comes from a patient with extra skin on the abdomen from having multiple children. The laxity of the skin in this area is very common after the stomach has been stretched multiple times from pregnancy. As you can see this woman would not be able to lose anymore weight or do any more crunches to improve her stomach. Her skin was not able to regain its tightness and elasticity.

A tummy tuck to remove this extra skin was performed allowing her to have a flat stomach once again. The photo shows her stomach before surgery and three months post operation.

In our second before and after plastic surgery photo features a patient that had about 5 pounds of tissue removed from the abdominal area to give her back a flat and more appealing stomach. Additionally in this patient a small amount of liposuction was performed to improve the contour.

Liposuction & Cosmetic Surgery: Why so popular?

Cosmetic Surgery and Liposuction are becoming more mainstream.

According to the American Society of Plastic Surgeons’ report on 2011 Plastic Surgery Procedural Statistics, 182,000 liposuction procedures were performed on women in America that year, while 23,000 were performed on men. Interestingly, the Men’s numbers dropped slightly from 2010 and the Women’s went up. More and more men seem to be going in for Face lifts, though. But today we’re going to discuss Liposuction: The best thing to happen to beauty & fitness, since Diet & Exercise.

History and origins of the concept of Liposuction.

It is thought that the first removal of fat by suction was performed in 1920s France by Charles Dujarier. Sadly, gangrene developed in his first patient, leading to the death a famous Parisian model. His career ended. Then in 1974, Italian gynecologist Dr. Giorgio Fischer started practicing liposuction procedures by vacuuming subcutaneous fat with a better eye towards sterile technique, and a completely new field of medicine was born.

Liposuction comes into it’s own.

In 1987, two enterprising French doctors named Illouz and Fournier refined the technique even further. They would slice the underlying fatty tissues with a rotating, electric scalpel, inserted through small incisions. The tool they used was basically an oscillating blade housed within a thin metal tube, called a cannula. It carved up fat cells, along with other tissues and fluids, which were then suctioned away via rubber tubing by light vacuum.

By 1980, liposuction had become extremely popular here in America. Most of the negative publicity was centered around excessive bleeding and rippling of the skin. Then in 1985, a Dermatologist named Dr. Jeffrey A. Klein invented the “tumescent” technique, completely revolutionizing liposuction surgery. Using much smaller cannulas, liposuction could now be performed under local anesthesia, with excessive bleeding and undesirable skin depressions or rippling, a thing of the past.

Fat Removed via Plastic Surgery Does Not Return

New Liposuction Study proves that removed fat is gone

Dr. Eric Swanson, a Member Surgeon of the American Society of Plastic Surgeons (ASPS), evaluated 301 patients undergoing liposuction alone, and liposuction in combination with abdominoplasty. Dr. Swanson employed pre and post surgical photographs in conjunction with computer-aided measurements. Patient body dimensions were measured, before and at least three months after surgery.

The study concluded that no fat regrowth was observed in treated areas of the arms, abdomen or lower body. All other upper body dimensions remained unchanged after surgery, as well. Fat cells were not redistributed, nor had they migrated.

“Previous studies have evaluated small numbers of patients and used imprecise measuring techniques, limiting their power to reliably detect changes,” Dr. Swanson said in an interview. “Our study included a sufficient number of patients and used precise measurements, making the conclusions highly reliable. Patients can be reassured that their improvements will last and they need not worry about putting weight back on disproportionately.”

No indicators that fat cells will regenerate

While a poor diet can of course create new fat cells, Dr. Swanson’s study also discovered that even a year or more after liposuction and combination liposuction / abdominoplasty procedures, the average reductions in hip measurements remained significant. Even a group of 34 patients who had gained five pounds or more up to a year after their surgeries, still maintained significant reductions. This indicates that long term expectations are high.

“Of course, liposuction does not change your ability to gain or lose weight.” Dr. Swanson continued. “Its value is in permanently reducing the number of fat cells in problem areas. For any given weight you will be in the future, your proportions will be better after liposuction, barring extremes in weight gain.”
