Understanding the Deep Plane Facelift and the Differences Between Facelift Types

Facelifts are a popular cosmetic solution for individuals seeking a youthful appearance. But what exactly is a facelift? Essentially, it’s a surgical procedure that rejuvenates the face by tightening sagging skin, repositioning the underlying structural parts of the face, smoothing out wrinkles, and restoring volume. Many consider it as an option to slow or reverse visible signs of aging, often driven by a desire for enhanced confidence and aesthetic harmony. With various options available, it’s crucial to choose the right type for your needs. Today, let’s delve into the deep plane facelift and compare it with other facelift types.

What is a Deep Plane Facelift Surgery?

A deep plane facelift is a surgical technique targeting deeper tissues and structures of the face. Unlike other types of facelifts, which can only focus on the skin, this method involves repositioning the deeper layers to enhance the overall structure. This allows for more natural-looking results and significant changes and is particularly effective for sagging cheeks and mid-face areas.

It can also be combined with a brow lift, eyelid lift, and neck lift for a more youthful appearance, as well as a more comprehensive facial rejuvenation.

Who is a Good Candidate for a Deep Plane Facelift?

Most individuals who are looking for significant improvements in their facial features and seeing visible signs of aging are good candidates for a deep plane facelift surgery. That said, there are some criteria we follow at JanigaMDs to determine the eligibility of each patient. Ideal candidates for the deep plane facelift technique must be in good overall health, be nonsmokers, have realistic expectations, and not have any medical conditions that could negatively affect the healing process.

When you receive a deep plane, the technique will differ slightly from patient to patient. Most patients are in their 50s or above when they have a procedure, but this can vary depending on your concerns and the results you’re looking for. The best way to know what treatments are best for your specific needs is to schedule a consultation with a board-certified surgeon, like Dr. Timothy Janiga, and discuss your goals and lifestyle.

How is a Deep Plane Facelift Performed?

In a deep plane facelift, the dissection goes beneath the SMAS, into the “deep” plane, releasing ligaments and attachments. Typically, the retaining ligaments like the zygomatic and masseteric ligaments are released for greater lift and natural movement. The skin and the SMAS are often and variably lifted together as a single unit. The deep plane allows for mobilization of deeper facial structures, including malar fat pads, to restore volume and contour in the midface. A deep plane facelift is more technically challenging, so if you are interested in this facelift option, you should meet with experienced board-certified plastic surgeons who have many years of experience with facelifts to determine which type is best for you.

Technical Differences from Other Facelift Types

Each facelift procedure differs in a few key areas, including technical aspects and elements. Some patients are better candidates for other facelift types versus the deep plane due to a variety of different reasons.

SMAS Facelift

Focused on tightening and repositioning the SMAS layer, including the facial muscles and facial skin, addressing the mid and lower face, the less invasive SMAS facelift usually results in less dramatic outcomes. The SMAS (superficial musculoaponeurotic system) is an anatomic layer below the skin and above the muscles of the face. In a standard facelift, the main focus is on the SMAS, which is either folded, plicated, trimmed, or sutured. The skin is re-draped, and any excess skin is removed. The muscles and deeper tissues are generally not released or repositioned beyond the SMAS. The goal is to improve the mid-face, jawline, and neck by lifting superficial tissues. This facelift is often done in patients with earlier facial aging changes.

Mini Facelift

Offering a less invasive option for those with early signs of facial aging (including excess skin and nasolabial folds), the mini facelift involves smaller incisions and less recovery time. It is generally best for individuals with limited sagging, but with more sagging or laxity that can be treated with nonsurgical options like fillers or skin tightening devices. As a result, this facelift option provides less significant improvements than a deep plane facelift. However, while the deep plane facelift may take up to 6 months to see your results, a mini facelift will take up to 3 months.

Mid-Facelift

Specifically targeting the cheek and mid-face region, the mid-facelift is often used as a complement to other procedures like eyelid surgery. This type of facelift targets specific areas, so it doesn’t provide an overall rejuvenation like a deep plan facelift will. Instead, the mid-facelift focuses on tightening skin laxity in areas like sunken or sagging cheeks or deepening nasolabial folds. Generally, this procedure results in lifting and repositioning underlying structures and tightening the muscles through the facial region.

Comparing Facelift Depth, Recovery, and Results

As each facelift option differs depending on various factors, some may be good candidates for one option and not good candidates for another. Analyzing each option individually can help provide pertinent data as to which treatment option is best for your needs.

Procedure Depth Recovery Time Longevity Ideal Candidate
Deep Plane Lift Deeper soft tissues 2-4 weeks 10-15 years Moderate to severe sagging, typically ages 50+
SMAS Facelift Superficial layer 2-3 weeks 5-10 years Early to moderate sagging and skin laxity, ages 40-60
Mini Facelift Superficial layer 1-2 weeks Up to 5 years Mild sagging, ages 30-50
Mid-Facelift Focal areas (cheeks) 1-2 weeks 5-8 years Targeted improvements, often younger candidates seeking specific changes

Comparing Standard Facelifts to Deep Plane Facelifts

Looking at the two most common facelift types, SMAS and deep plane, the technical differences are listed below. Deep plane facelifts, while more comprehensive and generally longer lasting, are substantially more involved and invasive, which can bring about its own challenges.
